This form is for local authority or care trust use only.

Use the online form or email to tell the Child Benefit Office as soon as it’s clear a child will be looked after by a local authority or care trust for 8 weeks or more.

There’s no need to tell the Child Benefit Office if a child is being looked after for less than 8 weeks.

You’ll need the:

child’s given names and any other names the child is known by date the child started being looked after by the local authority or Care Trust contact details of the social worker, locum, duty social worker or clerk attached to the child Related forms and guidance
