Updated: Small update to the declaration section.
To apply for the Basic Payment Scheme in England in 2015, you need to complete a BP5 application form. If you can’t fit all the information you need to onto your form, use one the continuation booklets on this page to put the extra information in.
The blank BP5 form will help you prepare for your claim. RPA will be sending out BP5 applications forms during April that they have already fill in, based on the information they hold.
