Updated: A new, interactive, version of the form has now been published which includes a page detailing payment options.
This form is used to apply for IVA for LGVs with a gross vehicle weight of up to 3,500kg. This form replaces the old IVA 1 form for LGVs.
All these kinds of vehicles need to be approved to make sure they meet certain safety and environmental standards.
Please read and understand the information provided in the IVA 1LG guidance notes above otherwise your application may be delayed.
For passenger vehicles that require a test please use IVA application form for passenger cars (IVA 1C).
